  cosmetic only

  Make "module name" actually match the file name.
  Invoke with ';' as leaving it off confuses Lindent and gcc doesn't care.
  Fix indentation where Lindent did get confused.

  initialised, with grave results.

  - Make it a mutex

  - Initialise it

  - Make it static
